The Global Fund is a global public/private partnership dedicated to attracting and disbursing additional resources to prevent and treat HIV/AIDS, tuberculosis and malaria. 61% of all international funding Global Fund provides to combat HIV/AIDS. As GF support is mainly directed towards low-income regions (Africa), the overall 9% of its support goes to Eastern Europe and Central Asia. Funding provided by Global Fund is one of the largest mechanisms supporting national health projects in 22 countries in the region.
When implementing Global Fund projects, key partners for national governments are civil society organizations (CSOs). Almost 60% of all grants in Eastern Europe and Central Asia are realized through such organizations. However, additional technical support is required for CSOs from the Global Fund in order to increase their capacity – which is why the Civil Society Action Team (CSAT) was launched in January 2008.
CSAT priorities are:
-    Mobilize a civil society response to under-performing grants;
-    Improve availability of quality technical support for civil society organizations involved in implementation of the Global Fund grants;
-    Improve the quality of Global Fund-related proposals prepared by civil society organizations.
EHRN is the regional host for Eastern Europe and Central Asia (global host is the International Council of AIDS Service Organizations (ICASO), whose International Secretariat is based in Canada).

The Civil Society Action Team (CSAT) for Eastern Europe and Central Asia (EECA)

 

 

The Civil Society Action Team (CSAT) is a civil society-led global initiative that coordinates, brokers and advocates for technical support to civil society organizations implementing or seeking grants from the Global Fund to Fight AIDS, TB and Malaria (the Global Fund).

 

Given the critical value of harm reduction, as a public health strategy, and the role of the community of injecting drug users as a key population, in responding to HIV/AIDS and TB in EECA through the Global Fund programs, in 2008 EHRN became a CSAT hub for the region of Eastern Europe and Central Asia.

Global Fund Executive Director responded to the open letter of networks and NGOs from EECA

In December 2012 18 networks and NGOs from Eastern Europe and Central Asia (EECA) have written to Mark Dybul, incoming Executive Director of the Global Fund, an open letter to express concerns about what they perceive as negative impacts of recent developments at the Fund.

On 25th of February Mr. Dybul has responded to this letter. In his response the Executive Director covers next areas of NGOs' concern:

Regional seminar “Getting involved with the Global Fund: how to address community systems and TB counteraction challenges in Eurasia”

 

November 20-22, 2012, Kyiv, Ukraine

 

 

 

 

The idea of organizing a workshop specific to the NGOs working as sub- and sub-sub recipients of the Global Fund to Fight AIDS, Tuberculosis and Malaria (the Global Fund) tuberculosis (TB) grants came after a successful workshop for NGOs implementing of HIV responses supported by the Global Fund in June 2012. Then few EHRN staff and experts suggested organizing a similar event for NGOs in the TB field. This suggestion was backed by an idea that so far there were few efforts specific towards TB NGOs supported through the Global Fund grants and TB voices are weak in the country and global level structures related to the Global Fund, while intensified responses to TB needed.

 

The regional seminar gathered 16 representatives from TB-affected community and civil-society organizations engaged with the Global Fund to review how improve their involvement as sub-recipients, sub-sub-recipients or members of Country Coordinating Mechanisms (CCMs). The geography of the participants covered 5 countries: Armenia, Belarus, Kyrgyzstan, Moldova and Ukraine. The seminar was held in the Russian language and followed a similar seminar in June 2012, which was focused on HIV.

 

Organized by the Eurasian Harm Reduction Network (EHRN) in partnership with  the regional hub of Civil Society Action Team (CSAT) and Harm Reduction Knowledge Hub (both hosted by EHRN) with the financial support of the GIZ, Stop TB Partnership and Open Society Foundations.

The regional seminar was facilitated by Ms. Raminta Stuikyte, European AIDS Treatment Group (EATG), independent consultant on public health, drug policy and civil society, and Dr. Nonna Turusbekova, independent consultant TBC Consult | HRM for Health. It was also supported by experts: Ms. Elena Zaytseva from the Global Fund’s Secretariat, Ms. Irina Zatușevski from PAS, an NGO taking the role of Principal Recipient in Moldova, Mr. Alexandr Curașov from the Communities Delegation to the Global Fund Board, and Mr. Ivan Varentsov from EHRN.

Regional workshop & training

"Getting involved with the Global Fund: how to address community systems and harm reduction challenges in Eurasia"

Istambul, 25 - 27 June, 2012

 

 

 

On June 25 – 27, a pilot training - seminar "The Global Fund reform and the meaningful participation of civil society in the Global Fund structures in the context of this process" was held in Istanbul (Turkey). The seminar was planned and organized by the EHRN within its Civil Society in Action Team (CSAT) initiative with financial support provided by GIZ, OSI and ICASO.

 

 

 

The purpose of the seminar was to support national and global solutions to problems experienced by harm reduction projects and NGOs involved in the Global Fund grant implementation in EECA as sub- and sub-sub-recipients by building their advocacy and managerial capacity. Also at the seminar the organizers set the task to raise awareness and increase understanding of SRs and SSRs representatives of the Global Fund structure, its mechanisms and processes at the country and global levels. 17 NGO representatives from 8 EECA countries attended the seminar, including representatives from Armenia, Belarus, Georgia, Kyrgyzstan, Moldova, Tajikistan, Uzbekistan and Ukraine. Report by Eurasian Harm Reduction Network (EHRN) developed in the lead-up to the 26th Global Fund Board meeting, which is taking place in Geneva on May 10-11, 2012. The report assesses the consequences of the sharp reduction in donor support that forced the Global Fund to halt new funding and to impose cost-cutting measures.


Over the last decade, the Global Fund has played a unique and indispensable role in responding to the HIV epidemic among people who use drugs in Eastern Europe and Central Asia (EECA). From 2002 to 2009, it approved $263 million for harm reduction in EECA alone—more than all other international sources combined.

 


The Global Fund made important investments in capacity building and advocacy efforts by civil society, but this work is far from complete; more, not fewer, funds are needed to build harm reduction capacity and political support, critical components in establishing sustainability. EECA, which is home to a fast-growing HIV epidemic concentrated among people who inject drugs (PWID), has already been affected and will be hit particularly hard by the recent Global Fund changes in the near future – please see Quitting While Not Ahead for details.  The Global Fund’s decisions to reduce funding availability based on country income level ignores the fact that income is not the determining factor for the availability of HIV services for PWID; rather, the decisive factor is political will. This is the wrong time for the Global Fund to reduce support for the HIV response in EECA, stepping away from sustainability and advocacy efforts on behalf of harm reduction. The need to protect harm reduction services from ideologically driven attacks and laws that directly or indirectly criminalise people who inject drugs makes it imperative to do more and better on critical enablers of harm reduction in the region.

At the 25th Board meeting of the Global Fund to Fight AIDS, Tuberculosis and Malaria (the Global Fund), held from November 21 to 22, 2011 in Accra (Ghana), it was decided to cancel the 11th round of grants due to the lack of funds.  At the same meeting, a decision was made to create the Transitional Funding Mechanism (TFM), designed to allocate limited available resources for the continuation of programs that provide essential services in the area of prevention, treatment and care and are currently supported by the Global Fund.

A main information on TFM is presented in the document, relevant examples from EECA are given, references on the main sources on information on TFM are given.

Regional Consultation

 

Responding to the Financial Crisis of the Global Fund in Eastern Europe and Central Asia

Aims of the Consultation

 

a) React to the problems with funding HIV and TB prevention and treatment programs in Eastern Europe and Central Asia (EECA) that emerged in the context of the decisions of the Global Fund Board’s 25th meeting;

b) Develop an action plan for regional advocacy in order to mobilize/redistribute/effectively use resources (based on an assessment of how the funding deficit at the Global Fund will impact the situation), to:

- Call upon the governments of developed countries to fulfil their financial commitments and increase their investments;

- Call upon the governments of EECA countries, particularly those in the middle-income level category, to increase domestic funding for healthcare, and to increase the effectiveness of their HIV programs;

Call upon the Global Fund Board to fulfil its commitment to Community Systems Strengthening and human rights protection as the basis for the Fund’s programs, and to support regional initiatives to ensure optimal effectiveness of their investments.

 

 

 

The consultation put forward the following tasks:

1. Prepare an overview of the Global Fund’s active grants, evaluate the impact of the decisions of the Global Fund Board’s 25th meeting on the HIV and TB situation in Eastern Europe and Central Asia, as well as analyze the risks of program closures or interruptions;

2. Become acquainted with the Global Fund’s latest recommendations regarding the new Transitional Funding Mechanism (TFM) which is designed to ensure the continuation of the Core programs, and regarding the reprogramming of the active grants in order to improve the efficiency of the Fund’s active projects;

3. Assess the perspectives for the Global Fund's replenishment process in 2012 ‐ 2014, and the EECA civil society’s role in resource mobilization;

4. Establish a consultative group for coordination, adjustment and implementation of the regional advocacy action plan.

You can do it! How to access resources from the Global Fund for Community Systems Strengthening: Guidance for communities of people who use drugs, men who have sex with men, sex workers and people living with HIV and other key affected populations in Eastern Europe and Central Asia

Since the HIV epidemic began, it has been clear that effective and sustainable responses depend on communities of people affected by the epidemic working alongside with, and in partnership with, governmental and private-sector initiatives. In countries with epidemics concentrated among most-at-risk populations (MARPs), as is the case in most of the countries of Eastern Europe and Central Asia (EECA), communities of affected populations play an especially important role. They are often the most vocal champions of policy change in support of evidence-based public health interventions and measures to protect human rights. They are uniquely able to reach out to injecting drug users (IDUs), men who have sex with men (MSM), sex workers, people living with HIV (PLHIV) and other key affected populations with services and are able to mobilize their own communities for behaviour change. The strengthening of community systems, which support and amplify the efforts of these groups, is a key element in a sustained response to the epidemic and achieving improved health outcomes. The Global Fund to Fight AIDS, Tuberculosis and Malaria (Global Fund) recognizes this fact and wants community-based initiatives to access its funds for their work. It has therefore developed and instituted a “Community Systems Strengthening (CSS) Framework” to enhance support of community-based initiatives within country proposals.

All Global Fund Round 10 country proposals from EECA countries contained elements of CSS, but to a lesser degree than most other regions in the world (Global Fund, 2011). Globally, key affected populations face many challenges in accessing funds despite their vital role in HIV responses. This guideline is intended to provide practical advice, tools and examples for civil society representatives from and serving key affected populations to assess needs for, develop proposals for and advocate inclusion of CSS components in proposals to the Global Fund. While this guideline focuses on the HIV proposals submitted through funding rounds, it can be useful for developing National Strategy Applications (NSAs), as well as TB proposals.

Meaningful and active participation of most-at-risk populations in combating epidemics of HIV, tuberculosis and malaria is one of the most important factors in making this combat effective. Therefore, to ensure participation of the most-at-risk populations in this process, development and implementation of a community system strengthening strategy is critical.

The Eurasian Harm Reduction Network (EHRN) in the framework of the International "Civil Society in Action Team" Initiative (CSAT) in collaboration with the East Europe and Central Asia Union of PLWH (ECUO) and the International Alliance on HIV/AIDS in Ukraine held the Third Regional Consultation "Community systems strengthening for key affected groups in Eastern Europe and Central Asia" on 22-24 August 2011 in Irpen, Ukraine.

To prepare for the consultation and select consultation participants, an organizing committee was formed; the organizing committee is comprised of representatives of the consultation organizers, their partners and donors. In accordance with selection criteria, people who use drugs (PWUD), men who have sex with men (MSM) and people living with HIV (PLHIV) and had an opportunity or planned to participate in the development of country proposals to the Global Fund to fight AIDS Tuberculoses and Malaria (the Global Fund) in the 11th Round as well as representatives of Country Coordinating Mechanisms, international organizations and UN agencies were invited to the consultation.

At the 23d Global Fund Board meeting a decision was made on the new eligibility criteria for countries willing to apply for financial support from the Global Fund to combat the three diseases. Below you will see analysis of possible implications of the new eligibility criteria for Eastern European and Central Asian countries’ eligibility to apply for HIV/AIDS funding in Round 11 of the Global Fund.

 

The analysis and its results provided in this document are in accordance with the final list of countries, including from EECA, that are eligible to apply for HIV/AIDS funding in Round 11, published by the Global Fund.

In the countries of Eastern Europe and Central Asia, significant progress in scaling up access to HIV prevention, treatment, care and support have been made through programs supported by the Global Fund to Fight AIDS, TB and Malaria. Civil society organizations throughout the world have play an important role in the planning, implementation and evaluation of GFATM programs as CCM members, principal recipients, and sub-recipients. Carefully planned, coordinated and monitored technical assistance can help to maximize the potential benefits of the civil society role though it is often inadequate.
