The Global Fund is a global public/private partnership dedicated to attracting and disbursing additional resources to prevent and treat HIV/AIDS, tuberculosis and malaria. 61% of all international funding Global Fund provides to combat HIV/AIDS. As GF support is mainly directed towards low-income regions (Africa), the overall 9% of its support goes to Eastern Europe and Central Asia. Funding provided by Global Fund is one of the largest mechanisms supporting national health projects in 22 countries in the region. When implementing Global Fund projects, key partners for national governments are civil society organizations (CSOs). Almost 60% of all grants in Eastern Europe and Central Asia are realized through such organizations. However, additional technical support is required for CSOs from the Global Fund in order to increase their capacity – which is why the Civil Society Action Team (CSAT) was launched in January 2008. CSAT priorities are:
- Mobilize a civil society response to under-performing grants;
- Improve availability of quality technical support for civil society organizations involved in implementation of the Global Fund grants;
- Improve the quality of Global Fund-related proposals prepared by civil society organizations.
EHRN is the regional host for Eastern Europe and Central Asia (global host is the International Council of AIDS Service Organizations (ICASO), whose International Secretariat is based in Canada).

The Civil Society Action Team (CSAT) for Eastern Europe and Central Asia (EECA)
The Civil Society Action Team (CSAT) is a civil society-led global initiative that coordinates, brokers and advocates for technical support to civil society organizations implementing or seeking grants from the Global Fund to Fight AIDS, TB and Malaria (the Global Fund).
Given the critical value of harm reduction, as a public health strategy, and the role of the community of injecting drug users as a key population, in responding to HIV/AIDS and TB in EECA through the Global Fund programs, in 2008 EHRN became a CSAT hub for the region of Eastern Europe and Central Asia.

At the 25th Board meeting of the Global Fund to Fight AIDS, Tuberculosis and Malaria (the Global Fund), held from November 21 to 22, 2011 in Accra (Ghana), it was decided to cancel the 11th round of grants due to the lack of funds. At the same meeting, a decision was made to create the Transitional Funding Mechanism (TFM), designed to allocate limited available resources for the continuation of programs that provide essential services in the area of prevention, treatment and care and are currently supported by the Global Fund.

Responding to the Financial Crisis of the Global Fund in Eastern Europe and Central Asia

Aims of the Consultation
a) React to the problems with funding HIV and TB prevention and treatment programs in Eastern Europe and Central Asia (EECA) that emerged in the context of the decisions of the Global Fund Board’s 25th meeting;
b) Develop an action plan for regional advocacy in order to mobilize/redistribute/effectively use resources (based on an assessment of how the funding deficit at the Global Fund will impact the situation), to:
- Call upon the governments of developed countries to fulfil their financial commitments and increase their investments;
- Call upon the governments of EECA countries, particularly those in the middle-income level category, to increase domestic funding for healthcare, and to increase the effectiveness of their HIV programs;
Call upon the Global Fund Board to fulfil its commitment to Community Systems Strengthening and human rights protection as the basis for the Fund’s programs, and to support regional initiatives to ensure optimal effectiveness of their investments.
The consultation put forward the following tasks:
1. Prepare an overview of the Global Fund’s active grants, evaluate the impact of the decisions of the Global Fund Board’s 25th meeting on the HIV and TB situation in Eastern Europe and Central Asia, as well as analyze the risks of program closures or interruptions;
2. Become acquainted with the Global Fund’s latest recommendations regarding the new Transitional Funding Mechanism (TFM) which is designed to ensure the continuation of the Core programs, and regarding the reprogramming of the active grants in order to improve the efficiency of the Fund’s active projects;
3. Assess the perspectives for the Global Fund's replenishment process in 2012 ‐ 2014, and the EECA civil society’s role in resource mobilization;
4. Establish a consultative group for coordination, adjustment and implementation of the regional advocacy action plan.

- Representatives of the leading regional and national networks, project organizations working in EECA countries, as well as government structures (from Albania, Belarus, Bosnia and Herzegovina, Georgia, Kyrgyzstan, Lithuania, Macedonia, Moldova, Russia, Ukraine, and Estonia)
- Members of the Global Fund Board and representatives of the following delegations: EECA countries, NGOs from developing and developed countries, private foundations;
- Representatives of the Global Fund Secretariat;
- Technical support agencies and technical partners (WHO, UNAIDS, UNDP, Open Society Foundations)

Meaningful and active participation of most-at-risk populations in combating epidemics of HIV, tuberculosis and malaria is one of the most important factors in making this combat effective. Therefore, to ensure participation of the most-at-risk populations in this process, development and implementation of a community system strengthening strategy is critical.

The Eurasian Harm Reduction Network (EHRN) in the framework of the International "Civil Society in Action Team" Initiative (CSAT) in collaboration with the East Europe and Central Asia Union of PLWH (ECUO) and the International Alliance on HIV/AIDS in Ukraine held the Third Regional Consultation "Community systems strengthening for key affected groups in Eastern Europe and Central Asia" on 22-24 August 2011 in Irpen, Ukraine.

To prepare for the consultation and select consultation participants, an organizing committee was formed; the organizing committee is comprised of representatives of the consultation organizers, their partners and donors. In accordance with selection criteria, people who use drugs (PWUD), men who have sex with men (MSM) and people living with HIV (PLHIV) and had an opportunity or planned to participate in the development of country proposals to the Global Fund to fight AIDS Tuberculoses and Malaria (the Global Fund) in the 11th Round as well as representatives of Country Coordinating Mechanisms, international organizations and UN agencies were invited to the consultation.

08.06.2011. Analysis of EECA countries that will be able to apply for HIV/AIDS in the framework of the Global Fund Round 11.
At the 23d Global Fund Board meeting a decision was made on the new eligibility criteria for countries willing to apply for financial support from the Global Fund to combat the three diseases. Below you will see analysis of possible implications of the new eligibility criteria for Eastern European and Central Asian countries’ eligibility to apply for HIV/AIDS funding in Round 11 of the Global Fund.
The analysis and its results provided in this document are in accordance with the final list of countries, including from EECA, that are eligible to apply for HIV/AIDS funding in Round 11, published by the Global Fund.

In the countries of Eastern Europe and Central Asia, significant progress in scaling up access to HIV prevention, treatment, care and support have been made through programs supported by the Global Fund to Fight AIDS, TB and Malaria. Civil society organizations throughout the world have play an important role in the planning, implementation and evaluation of GFATM programs as CCM members, principal recipients, and sub-recipients. Carefully planned, coordinated and monitored technical assistance can help to maximize the potential benefits of the civil society role though it is often inadequate. This report aims to draw attention to this often neglected though vital issue.
